New Patients

Loren, Jessica and the entire team were incredibly professional and friendly. They actually made me feel like I wanted to come to physical therapy. And I can't say that for all PT partners I've chosen in the past! 

The other good part is that when My back started feeling better/stronger, and they knew I could do the exercises at home, they graduated me. They didn't keep asking me to come back for appointments to increase their profits. They were honest and told me I needed to be a "big girl" and do it on my own! And they gave me the tools and the knowledge to do so.

What to expect on the first visit
The first visit will include the initial evaluation.  This is important in gathering all pertinent information that will help us provide you with the best possible treatment to meet your goals.

What is included in the first visit?
A complete history of your injury, areas of concern and personal goals for PT
A comprehensive biomechanical evaluation
Initial treatment as appropriate
Short home program as appropriate


How should I prepare for the first visit?
Please arrive 15 minutes prior to your appointment time to fill out and/or sign the requested intake forms.  For your convenience, you may print out the initial intake form here to fill out prior to your arrival.

Please present your insurance card to the front desk
Please bring any relevant X-rays, MRI’s or test results if you have access to them.  These may be helpful and sometime necessary in directing your evaluation and treatment.  


What should I wear?
Please bring or wear appropriate attire (i.e. shorts, sweats, t-shirts, etc.) that will allow access to your area of concern

I decided to go to KODA Physical Therapy on the advice of my orthopedic doctor who said Loren was one of the best physical therapists in Petaluma. I have not seen others in Petaluma, but I can say that I have had an excellent experience here.

 

Loren is great at explaining how your body works and the reasons for doing certain exercises. The building and equipment are really nice and he definitely will take as much time with you as you need. He also gets the job done and doesn't require you to come back for more therapy than is needed.

It's really nice being able to trust your therapist when doing certain exercises.
